2025 Tackle Football Registration

The following documentation is due by equipment distribution:

  • Football equipment deposit check of $500 (check will not be cashed unless equipment is not returned)
  • Volunteer deposit check of $500 (check will not be cashed unless volunteer hours are not completed)
  • 2025 Medical Clearance Form (must be dated after 4/15/25)
  • Copy of birth certificate 
  • Copy of end of year report card

Photo of player must be entered at time of registration. Picture must include player headshot only. 

Athlete must be registered ONLY to the team associated with their age (age as of June 15, 2025)
